VLDL (very-low-density lipoprotein) cholesterol is a type of 'bad' cholesterol whose main job is to carry triglycerides — a form of fat — through your blood. Because it's difficult to measure directly, most labs estimate it as your triglyceride level divided by five. A normal VLDL cholesterol is roughly 2–30 mg/dL; a high VLDL usually reflects elevated triglycerides and adds to cardiovascular risk.

What high VLDL can mean
- High triglycerides (the main driver, since VLDL carries them)
- Diet high in refined carbohydrates, sugar, or alcohol
- Obesity or metabolic syndrome
- Poorly controlled diabetes
- Hypothyroidism or kidney disease
What low VLDL can mean
- A low VLDL is generally not a concern and is favorable for heart health
- Very low values are usually unremarkable and can reflect very low triglycerides
What to do about an abnormal result
- Compare your value to the range on your own report — labs estimate VLDL differently.
- Because VLDL tracks triglycerides, the same steps that lower triglycerides (less sugar and alcohol, more activity, weight loss) lower VLDL.
