LDL cholesterol ("bad" cholesterol) is the cholesterol carried by low-density lipoprotein particles, which can build up in artery walls over time; on most lab reports it isn't measured directly but calculated from your total cholesterol, HDL, and triglycerides (the "LDL chol calc" line). An optimal LDL is below 100 mg/dL, borderline-high is 130–159 mg/dL, and 190 mg/dL or higher is very high; higher levels generally suggest greater cardiovascular risk, though reference ranges vary by lab and your personal target is set by a clinician based on your overall risk profile.

What high LDL can mean

  • A larger number of artery-clogging (atherogenic) particles and higher long-term cardiovascular risk
  • Diet high in saturated or trans fats
  • Genetics, including familial hypercholesterolemia (especially when LDL is 190 mg/dL or higher)
  • Obesity, physical inactivity, or poorly controlled diabetes
  • Hypothyroidism, kidney disease, or certain medications

What low LDL can mean

  • A low LDL is generally favorable for heart health and is often a treatment goal rather than a problem
  • Very low levels are occasionally seen with malnutrition, hyperthyroidism, liver disease, or certain genetic conditions
  • People taking lipid-lowering medication may intentionally have very low LDL
