WBC, or white blood cell count, measures the infection-fighting cells circulating in your blood and is part of every complete blood count (CBC). A normal adult white blood cell count is roughly 4,000–11,000 cells per microliter (cells/µL), equal to 4.0–11.0 × 10⁹/L. A high WBC (leukocytosis) usually reflects infection, inflammation, or physical stress on the body, while a low WBC (leukopenia) can mean reduced ability to fight infection — but reference ranges vary by lab, and an abnormal result is best interpreted by a clinician alongside your symptoms and the rest of your CBC.

What high WBC can mean

  • A bacterial or viral infection — the most common cause of a high white blood cell count
  • Inflammation from a chronic condition such as rheumatoid arthritis or inflammatory bowel disease
  • Physical or emotional stress, intense exercise, smoking, or even a recent meal, which can transiently raise WBC
  • Certain medications, especially corticosteroids, which raise WBC without an infection being present
  • A reaction after surgery, injury, or childbirth
  • Less commonly, a blood cancer such as leukemia — more likely when the count is very high, rises without an obvious cause, or comes with abnormal-looking cells on the differential

What low WBC can mean

  • Certain viral infections, which can transiently suppress white blood cell production
  • Medications, especially chemotherapy, some antibiotics, and certain psychiatric or thyroid drugs
  • Autoimmune conditions that attack white blood cells or the bone marrow
  • Vitamin B12, folate, or copper deficiency
  • Bone marrow disorders that impair white blood cell production
  • An enlarged spleen, which can trap and remove white blood cells from circulation

What to do about an abnormal result

  • Compare your result to the reference range printed on your own report — cells/µL cutoffs vary by lab and can shift slightly with age.
  • A mildly high WBC found during a cold, minor infection, or right after exercise is often just rechecked once you've recovered, rather than treated as its own condition.
  • A markedly high (over roughly 30,000 cells/µL) or notably low white blood cell count — especially with fever, unusual bleeding, or feeling unwell — should be evaluated by a doctor promptly.
  • If your WBC is low, ask your doctor about the differential and absolute neutrophil count (ANC): neutropenia, the actual infection-risk threshold, is defined by ANC rather than total WBC, and severe neutropenia generally means an ANC under 500 cells/µL.
