Immature granulocytes (IGs) are developing infection-fighting white blood cells—mainly promyelocytes, myelocytes, and metamyelocytes—that usually remain in bone marrow. Healthy adults commonly have about 0–0.5% IGs, although some laboratories use an upper limit below 1%, and results may also appear as an absolute count. A high result can reflect infection, inflammation, physical stress, or bone-marrow recovery, but it does not diagnose any condition by itself; reference ranges vary by lab, analyzer, and age.

What high Immature Granulocytes can mean

  • A bone-marrow response to an acute infection or inflammation; the result supports context but cannot identify the cause or prove a bacterial infection
  • Major physical stress, tissue injury, surgery, or another acute illness
  • Bone-marrow recovery after suppression, including recovery after some infections or treatments
  • The newborn period, when expected values differ from adult and older-child ranges
  • Less commonly, a bone-marrow or blood disorder—especially when the elevation persists or accompanies other abnormal counts or cells

What low Immature Granulocytes can mean

  • Zero or a very low immature granulocyte result is expected in many healthy adults
  • There is generally no clinically meaningful 'too low' immature granulocyte count in an otherwise reassuring CBC
  • A low IG value does not rule out infection; symptoms and the broader clinical picture still matter

What to do about an abnormal result

  • Check whether your report gives IG as a percentage, an absolute count, or both, and compare the result with that laboratory's reference range.
  • Review the total WBC and absolute neutrophil count (ANC) separately: total WBC counts all white cells, while ANC counts circulating neutrophils; neither is interchangeable with IG%.
  • Do not treat a mildly high IG result as proof of bacterial infection or as a reason to take antibiotics without a clinician's evaluation.
  • Ask your healthcare provider whether a repeat CBC, a manual blood-smear review, or other testing is appropriate if the result is persistent, rising, or accompanied by other abnormal blood counts.
  • Seek prompt medical care for concerning symptoms such as fever, trouble breathing, confusion, or feeling severely unwell; urgency is based on symptoms and the full CBC, not the IG value alone.

What does a high immature granulocyte percentage mean?

A high immature granulocyte percentage means the bone marrow is releasing more early granulocyte forms into the bloodstream than the laboratory expects. This can happen with infection, inflammation, major physical stress, tissue injury, or marrow recovery, and less commonly with a blood or bone-marrow disorder. The result is interpreted with the absolute IG count, total WBC, ANC, symptoms, and sometimes a blood smear; it is not a diagnosis by itself.

What is a normal immature granulocyte range?

Many adult reports use an IG percentage around 0–0.5%, while some use an upper limit below 1%; one published analyzer evaluation used up to 0.54% and 0.10 × 10³/µL as its local limits. These are examples rather than universal cutoffs because analyzers, populations, age, and pregnancy affect the interval. Use the reference range printed on your own report.

Are immature granulocytes the same as bands?

No. Bands are slightly immature neutrophils, whereas an automated IG count generally includes earlier forms such as promyelocytes, myelocytes, and metamyelocytes. Some ANC calculations include mature neutrophils plus bands, but bands may be reported separately and are not always included in the analyzer's IG value.

Do high immature granulocytes mean sepsis or leukemia?

No. An elevated IG result can occur during infection or severe illness, but studies show that IG alone is not accurate enough to confirm bacteremia or sepsis, and it does not diagnose leukemia. Persistent or marked elevation—especially with abnormal total counts, blasts, anemia, low platelets, or concerning symptoms—deserves clinician review and may prompt a manual smear or additional testing.
