Absolute neutrophils, usually reported as the absolute neutrophil count (ANC), measure the number of circulating neutrophils rather than their percentage of total white blood cells. A common adult reference interval is about 1,500–8,000 cells/µL, but ranges vary by lab and population. A high ANC often accompanies infection, inflammation, stress, or steroid use; an ANC below 1,500 is commonly called neutropenia, and below 500 is severe neutropenia with substantially higher infection risk.

What high Absolute Neutrophils can mean

  • An acute infection, often bacterial, or an inflammatory response; ANC alone cannot identify the organism or confirm infection
  • Physical stress, tissue injury, surgery, intense exercise, smoking, or pregnancy
  • Corticosteroids and certain other medicines, which can raise circulating neutrophils
  • Less commonly, a persistent inflammatory, blood, or bone-marrow disorder—especially when other CBC values are abnormal

What low Absolute Neutrophils can mean

  • A recent viral infection or, sometimes, a severe systemic infection
  • Chemotherapy, radiation therapy, or another medication that reduces production or increases destruction of neutrophils
  • An autoimmune condition or an enlarged spleen
  • Vitamin B12, folate, or copper deficiency
  • A bone-marrow disorder or an inherited neutropenia
  • A stable lower baseline associated with the Duffy-null red-cell phenotype in some people, which may not carry the infection risk implied by older population-wide cutoffs

What to do about an abnormal result

  • Confirm the unit and compare your ANC with the reference range on your own report; 1.5 × 10⁹/L is the same as 1,500 cells/µL.
  • Interpret ANC separately from total WBC and neutrophil percentage. ANC is an absolute count and is often calculated as WBC × (mature neutrophil percentage + band percentage) ÷ 100; immature granulocytes are not automatically interchangeable with bands.
  • Discuss a new or persistent low ANC with your clinician, who may review medications, recent infections, prior results, ancestry-related reference considerations, and the other CBC cell lines before deciding on repeat testing.
  • If you know you have neutropenia and develop a temperature of 100.4°F (38°C) or higher, contact your treating team or seek immediate medical care, particularly during chemotherapy or when ANC is below 500 cells/µL.
  • Do not start antibiotics, stop a prescription medicine, or use supplements to change ANC without medical guidance; treatment depends on the cause rather than the number alone.

What is a normal absolute neutrophil count?

Exact limits vary by laboratory, age, clinical setting, and population, and some healthy people have a stable count below 1,500 without increased infection risk. Compare your value with the range on your report and with your prior results.

What is the difference between neutrophil percentage and absolute neutrophils?

Neutrophil percentage is the share of all white blood cells that are neutrophils, while ANC is the actual estimated number of neutrophils per microliter. A percentage can look normal when total WBC is low, or high when another white-cell type is low, so the absolute count is generally more useful for assessing neutropenia and infection risk.

How is ANC calculated, and do bands count?

A common formula is total WBC × (percentage of mature neutrophils plus percentage of bands) ÷ 100. For example, a WBC of 4,000/µL with 40% neutrophils and 5% bands gives an ANC of 1,800/µL. Laboratory methods differ, and an automated immature granulocyte percentage usually represents earlier forms—not simply bands—so use the ANC reported by your laboratory when available.

When is a low ANC dangerous?

An ANC below 500 cells/µL is severe neutropenia and carries a substantially higher risk of serious infection, although risk also depends on how long the count stays low and the person's overall health. Fever of 100.4°F (38°C) or higher in someone known to be neutropenic needs immediate medical attention, especially during chemotherapy. A mildly low result without symptoms is often confirmed and evaluated rather than diagnosed from one test.

What can cause a high absolute neutrophil count?

A high ANC, called neutrophilia, commonly occurs with infection, inflammation, physical stress, tissue injury, smoking, pregnancy, or corticosteroid use.
