Mono Blood Test Results Explained: Monospot and EBV Antibodies

Mononucleosis (mono) blood tests look for the body's response to the Epstein-Barr virus (EBV). The quick monospot test detects heterophile antibodies, and a complete blood count often shows a high number of lymphocytes, including atypical ones. A positive monospot supports mono, but the test can be falsely negative early in the illness.

How blood tests are used

Mono is usually caused by EBV. The monospot is a fast screening test, but it's less reliable in the first week of symptoms and in young children. When results are unclear, specific EBV antibody tests provide a more detailed and accurate picture.

Why is my monospot test negative if I have mono symptoms?

The monospot test can be falsely negative, especially in the first week of illness or in young children, because heterophile antibodies take time to develop. If symptoms strongly suggest mono, doctors may repeat the test or order specific EBV antibody tests, which are more accurate.

What do EBV antibody results mean?

A VCA-IgM positive result with a negative EBNA suggests a recent or current EBV infection (active mono). VCA-IgG and EBNA both positive indicate past infection and immunity, not current illness. This panel helps distinguish a new infection from a previous one.

Can a blood test show if mono has affected my liver?

Yes. Mono can cause temporary liver inflammation, so liver enzymes (ALT and AST) are often mildly elevated. These usually return to normal as you recover, but your doctor may monitor them.
