Estradiol (E2) is the main form of estrogen, and a blood test measures it to assess ovarian, adrenal, or (in men) testicular hormone production. Levels vary enormously by menstrual cycle phase in premenopausal women (roughly 15–350 pg/mL, approximate and assay-dependent), are typically under 30 pg/mL after menopause, and run about 10–40 pg/mL in adult men. High estradiol can reflect pregnancy, fertility medications, or, rarely, a hormone-secreting tumor, while low estradiol can reflect menopause, primary ovarian insufficiency, or low body fat; because a single result can't be interpreted without knowing cycle timing, it's always read by a clinician alongside your cycle phase and symptoms, not used to diagnose anything on its own.

What high Estradiol can mean

  • Pregnancy (expected and often very high)
  • Ovulation induction or other fertility medications
  • An estrogen-secreting ovarian, adrenal, or (rarely) testicular tumor
  • Obesity, since fat tissue converts other hormones into estradiol (aromatization) — in men this can also be associated with gynecomastia (breast tissue enlargement)
  • Certain liver conditions that reduce estrogen clearance

What low Estradiol can mean

  • Menopause or perimenopause (the most common cause in women over 40)
  • Primary ovarian insufficiency (early loss of ovarian function)
  • Hypothalamic amenorrhea — often from low body fat, excessive exercise, or significant caloric restriction
  • Pituitary or hypothalamic conditions that reduce the hormone signals driving ovarian or testicular estrogen production
  • In men, low estradiol has been linked to lower bone density over time, since estrogen plays a role in male bone health — though a single low result doesn't by itself establish a bone health problem

What to do about an abnormal result

  • Note the day of your menstrual cycle (or menopause status) when the blood was drawn — estradiol is only meaningful when interpreted against cycle timing.
  • Compare your value to the range on your own report; reference ranges vary by lab and assay, and the ranges above are approximate.
  • In a fertility workup, estradiol is usually interpreted together with FSH, LH, and sometimes AMH, not as a stand-alone result.
  • Unexplained irregular periods, or estradiol that's markedly high or low for your expected cycle phase or age, should be discussed with your doctor rather than self-interpreted.

In men, low estradiol has been linked to bone health over time. Your doctor typically checks other hormones like FSH and LH alongside estradiol to help pinpoint the cause.

Can men have high estradiol?

Yes. In men, high estradiol is often associated with obesity, since fat tissue converts other hormones into estradiol, and it can be associated with gynecomastia (enlarged breast tissue).
